All medications administered at school (prescription and non-prescription) will need to be brought to the health office and be accompanied by a note from BOTH the parent and a physician. Download the Authorization for Adminstration of Medication Form.

Prescription medication needs to be brought to school in the original bottle from the pharmacy with the current label on. Non-prescription medication needs to come in the original bottle. We cannot give medication without parental consent and Physician signature.
